To answer your questions:
1.) Yes, I followed the bag instructions exactly. I measured the amount of water in a plastic container, then marked the container so that mixing was consistent.
2.) There is a lot of rebar in the slab. According to advice on this forum, the slab should actually have a %1 grade to drain off water. I poured it so that it had a %1 grade. It's 3.5" thick on one side, and about 5" thick on the other. There is more rebar towards the thin side than the thick side.
3.) Sure I can take photos. I'm at work now, but I'll post some macro shots of the cement. It seems plenty hard, and it developed an almost white color after a day or so.
