when you build the hearth slab, how do you guys get the cement up there?

    lifting

Unless you are willing to build a staging area above the hearth slab (and you still have to carry the dry ingredients up) you are just left with lifting it up bucket by bucket.

It's too small a quantity to get in a cement pump, which would solve the problem.

      It isn't that bad

It isn't terrible getting the hearth poured. You can do it with large plastic buckets (Home Depot sells them), shovel fulls, or two guys can lift a wheelbarrrow. You'll get tired, but I can't think of an easy mechanical means that would be worth the effort.

      Any other ideas?

I grabbed a couple of friends and we lifted wheelbarrow loads up into the forms. We poured the walls and the hearth this way.
